In-person learning is expected to begin on October 12
All new students must provide the following documents:
Proof of Residency
Emergency contact info - Names and phone numbers
Previous School Information - Name and phone number
CIB if Native American
Court Orders if applicaple
Official High School Transcript mailed to PO Box 519, Grand Canyon, AZ 86023, Att: Registrar

506 Form - Required for Native American students ONLY
Forms and documents may be emailed to email@example.com, faxed at 928-638-2045 Att: Registrar, or you may drop them off at the school office, Mon-Thu from 8am to 4pm.
